or canopies with cumulative areas of greater than 400 sq. ft? ( ) Yes ( ) No
(Please note if you are using multiple tents, if they are closer than 10 feet from each other they will be considered as one.) - Will there be any kind of open flame used (candles, torches, cooking devices, pyrotechnics)? ( ) Yes ( ) No
- Will compressed gas cylinders be used for vendor booths, cooking devices or any other function? ( ) Yes ( ) No
- Will the event be located in a roadway, pathway or plaza that used by emergency vehicles for access to areas
on campus? ( ) Yes ( ) No - Will the event include a barbecue or grill? ( ) Yes ( ) No
(If you answer yes to this question you need a Barbecue Permit from the Fire Prevention Division) - Will you be selling, preparing, or giving away food at this event? ( ) Yes ( ) No
(If you answer yes to this question you need a Food Permit from the Office of Environment, Health and Safety) - Will the event include alcoholic beverages? ( ) Yes ( ) No
(If you answer yes to this question you need a permit from the UCPD)
